Symptoms
When attempting to download an attached file after have used File Browse to upload it the following error occurs in the APEX run time environment:
The issue can be reproduced at will with the following steps:
1. Build an application page with a File Browse item.
2. Run the application and Upload a file.
3. Try to download the file using the download button.
The same application and download function works fine in a development environment.
