Patch version not reflected for Fusion Middleware component in Enterprise Manager

In Enterprise Manager ( EM ) after patching a Fusion Middleware ( FMW ) component it is observed that the version is still the old version. The patch ran successfully, but the version reported in EM is not in sync with the version that was patched.
Upgraded the OID/OVD SOFTWARE to 220.127.116.11 and then ran the PATCH SET ASSISTANT to upgrade the schema.
Everything got upgraded and the versions indeed showed 18.104.22.168 in schema_version_registry as well as ODSM console. But EM still showed 22.214.171.124."
Just tried to re-run the 126.96.36.199 patch installer. It complained that the patch had already been applied to this Oracle_Home. So, now I am perplexed. Let try to rebooting the box and restarting the WLS and OIF services.
"Interestingly, after the reboot the OIF version is still showing as 188.8.131.52 . So the patch is already applied, but the version reflected is not correct."
