Symptoms
In an FCKEditor field when image insert icon is clicked, in the file upload dialog, when file is picked and upload is clicked, user sees a message: "Upload a new file in this folder (Upload a new file in this folder (Upload in progress, please wait...)" which just stays there and the file doesn't get uploaded.
