Blank Page When Attempting to Access APEX After Configuring APEX Listener / Oracle REST Data Services (ORDS)

APEX has been configured with the APEX Listener / Oracle REST Data Services (ORDS). When attempting to access either the APEX administration pages or the APEX development environment, the user sees a blank page instead of the login page.
This problem can occur after the initial configuration of the APEX Listener / Oracle REST Data Services (ORDS) or after an APEX upgrade.
