Unable to log in using WNA, user account locked or disabled after adding LBR or proxy

When attempting to authenticate using a WNA authentication module, the user receives an error in the browser "User Account is Locked or Disabled" after configuring the OAM credential collector to be accessed via a load balancer or proxy.
