Portal URL /pls/portal Giving HTTP-404 Error, But /portal/pls/portal URL Working Fine. Also EM Is Reporting Portal:portal As Down

A 10.1.4.2 Portal is working fine when using URLs of the format http://host:port/portal/pls/portal.
An HTTP-404 message is displayed when http://host:port/pls/portal format is used.
A "hello world" procedure works fine, as long as the portal/pls/portal format is used eg http://host:port/portal/pls/portal/hello
EM is showing Portal:portal as down, but clicking on the link shows all components are up and running.
When attempting to go to the /pls/portal URL the following error is seen in the logs :-
