OIF 11g: logout / login results in 500 Internal Server Error (Doc ID 1462809.1)

Symptoms

 OIF is configured as service provider with Siteminder as Identity Provider. When we logout of the application and try to login again to the same application in the same browser session with a different userId, it results in Error 500 Internal Server Error.
