GlassFish Not Responding and Causes 100% CPU Utilization

Symptoms
GlassFish server is not responding and is hung. When checking the GlassFish Java process CPU usage, it shows 100% utilization.
From the Java thread dump captured using " kill -3 " that shows in the jvm.log, there are a lot of threads stuck in " java.util.HashMap.getEntry ".
