Symptoms

ISW used to syncronize but is now failing when trying to create a new user although modify and delete seem to be working without issue.  The ISW logs show ldap error code 19 which normally means there is a schema violation.

DS access logs:

[10/Jul/2012:13:47:00 -0500] conn=346922 op=2681 msgId=174888 - ADD dn="uid=<User.1>,cn=users,dc=<SUFFIX>"
[10/Jul/2012:13:47:00 -0500] conn=346922 op=2681 msgId=174888 - RESULT err=19 tag=105 nentries=0 etime=0, Password already hashed. Cannot check quality.

Corresponding ISW connector error log:
[10/Jul/2012:13:47:00.042 -0500] WARNING 244  CNN101 <HOST2>.<DOMAIN>.com  "Cannot create user 'uid=<User.1>,cn=users,dc=<SUFFIX>' at ldap://<HOST>.<DOMAIN>.com:<PORT>, ldap error code = 19." (Action ID=CNN102-1384D50E729-48529, SN=8)

and lastly the ISW connector audit log:
[10/Jul/2012:13:47:00.040 -0500] INFO    244  CNN101 <HOST2>.<DOMAIN>.com  "LDAP operation on entry uid=<User.1>,cn=users,dc=<SUFFIX> failed at ldap://<HOST>.<DOMAIN>.com:<PORT>, error(19): Constraint violation (Password already hashed. Cannot check quality.)." (Action ID=CNN102-1384D50E729-48529, SN=7)
