OAM 10g: Access To Any Protected Resource Gives Error "The WebGate plug-in is unable to contact any Access Servers." And Access Server Crash

Oracle Access Manager (OAM) 10.1.4.3 has been configured to protect application resources for Single Sign-On (SSO).
When any protected resource is accessed, after submitting credentials in the OAM form-based login page the following error is displayed:
When this error occurs, the Access Server process crashes.
The problem does not reproduce if the OAM Basic Over LDAP authentication scheme is used. It only reproduces when an OAM form-based authentication scheme is used.
Steps to reproduce
1. Access the protected application: the form-based login page is displayed.
2. Submit credentials: user is redirected to http://app.domain.com/oberr.cgi?status%3D500%20errmsg%3DErrEngineDown%20p1%3DFormAuthScheme%20p2%3DtxtUserID%253dtestuser1%2520pwd%253dtest%2520Resource%253d%2Fmysite%2F%2520RequesterIP%253d10.10.10.10%2520HostTarget%253dhttp%3A%2F%2Fapp.oracle.com%2520Operation%253dPOST
3. Error "The WebGate plug-in is unable to contact any Access Servers." is displayed in the browser.
