You have enabled "Managed Attachments" for Oracle E-Business Suite OAF Applications as per the instructions in the Oracle® WebCenter Administrator's Guide for Application Adapters 11g Release 1 (11.1.1) - Part Number E17953-02: http://docs.oracle.com/cd/E23943_01/doc.1111/e17953/c06_confg_attchmts.htm#ADTAD728

or, you used this Note:  How to Setup EBS 12.1.3 / 12.2.x OAF with Webcenter Content AXF Managed Attachments (Doc ID 1927793.1)

Calling the Managed Attachments URL from within the EBS Forms works fine, but when clicking the "Managed Attachments" button on the OAF web page nothing happens. Alternatively, an error page appears with the message: "Failed to communicate with the AXF server".
