Second OAM Server Fails Authentication With UCP Error (Doc ID 1503439.1)

Symptoms

Two OAM servers in a cluster have been working for a couple of months but now. Suddenly the second server oam-server2 is failing authentication with the following error:

oracle.security.am.engines.common.identity.provider.exceptions.IdentityProviderException
Caused by: oracle.security.idm.OperationFailureException: oracle.security.am.common.jndi.ldap.PoolingException [Root exception is oracle.ucp.UniversalConnectionPoolException: Invalid life cycle state. Check the status of the Universal Connection Pool]

Telnet connections from the server on the 10636 port work just fine, so it isn't a network problem. There is no firewall between OAM servers and OVD and OAMserver1 works just fine. Only the second server is failing.
