Symptoms
In the Content Presenter's in-context edit mode, should a file (HTML) be updated (in RTE) while simultaneously updating the file's metadata (Info - Advanced - Edit), existing tags are removed/replaced and/or new tags added at the top of the document.
1. The following:

is replaced with

2) The following is added at the top of the page:
