SOA 11g: soa-infra Activation Problem Due to NameNotFoundException with XmlSchemaChangeNotificationTopic (Doc ID 1505354.1)

Symptoms

In a 2 node SOA FMW 11.1.1.4 cluster, both of the soa managed servers start, but soa-infra stays in failed state. The following error shows in the diagnostic log file:
