OAM 10g: Access to Protected Resource Fails With "Oracle Access Manager Operation Error" (Doc ID 1505441.1)

Symptoms

Oracle Access Manager (OAM) 10g Single Sign On (SSO) is not working for access to an application.

When the application is accessed the following error is displayed in the browser:

Oracle Access Manager Operation Error

The mapping of credentials (userid=testuser Resource=/app/start RequesterIP=10.10.10.128 Host Target=http://app.oracle.com Operation=GET) to a user profile failed. The Access Server may not be able to connect to the user directory, or the authentication scheme TestFormAuthnScheme may have an invalid obMappingFilter parameter for its credential_mapping plugin.
