Symptoms
You have a clean installation of GlassFish Server 3.1.2.2 that was installed from the GlassFish 3.1.2.2 full profile installation bundle. This issue should not impact installations of GlassFish Server that have been patched to 3.1.2.2, which are then being subsequently patched to later release.
When you attempt to patch the installation you see the follow error both from the updatetool:
