New Case Screen for CSR Cases Does Not Display USER_LOGIN Or USER_ID Fields

USER_LOGIN or USER_ID fields are not visible while creating CSR type cases, how to enable these fields? Unable to create a new CSR case because the username field is missing from the Create New CSR Case window. Redeployment of OAAM Admin does not help.
