OAM 10g Form Login Fails With 404 Not Found "The requested URL .... was not found on this server." (Doc ID 1512576.1)

OAM 10g form-based login is failing. When credentials are submitted in the login page an error HTTP-404 Not Found occurs in the browser. The credentials are known to be valid.

Steps to reproduce

1. Access OAM-protected resource.
2. The OAM form login page is displayed.
3. Submit valid OAM credentials: 404 Not Found error occurs with a message similar to "The requested URL /auth/dummy.cgi was not found on this server."
