Oracle HTTP Server Fails to Start With the Error "Failed to open the wallet" After Wallet Creation or Renewing Certificate

Oracle HTTP Server (OHS) is failing to start with various errors (depending on Operating System) after a new wallet has been created or an expiring certificate is replaced. It is more common when an expiring certificate is being replaced. OHS was seen to fail with the following error messages:
The wallet can even be in ready status in Oracle Wallet Manager (OWM) appearing as though it is ready to be used.
Something has changed on the Certificate Authoritie's (CA) end where a new intermediate and/or root certificate is needed for the wallet.
