SOA 11g: Duplicate Records Getting Inserted into Tables by the Database Adapter (Doc ID 1526546.1)

Symptoms

Composite has an inbound File Adapter which picks up the file, and inserts records from the file into a database table using the Database Adapter.
Database adapter calls a PL/SQL package which inserts records into the table.

Duplicate records are seen in the table.

Changes

Database session corresponding to the Database Adapter had been killed by database administrator.
