JBO-29000: Clicking on My Access Tab After Upgrade to 11.1.2.0 (Doc ID 1529275.1)

Symptoms

After upgrading from OIM 11.1.1.5.x to OIM 11.1.2.0.0, when a pre-existing OIM user attempts to access the "My Access" tab then they see the error:

 
Also, in the OIM server logs you'll see:
