SQL errors in the logs that only gets resolved after a restart, but comes back after some time

Symptoms
Lots of SQL errors in the log file that look like this:
Caused by: javax.resource.ResourceException: No ManagedConnections available within configured blocking timeout ( 30000 [ms] )
at org.jboss.resource.connectionmanager.InternalManagedConnectionPool.getConnection(InternalManagedConnectionPool.java:303)
at org.jboss.resource.connectionmanager.JBossManagedConnectionPool$BasePool.getConnection(JBossManagedConnectionPool.java:575)
...
This caused publishes to fail and pages don't render correctly.
Only way to resolve this temporarily is to restart the application server, but the issue comes back after some time.
