API Throws Data Access Error After SOA Approval Workflow Completion

Scenario: One has written a process task adapter on a Connector which updates an attribute back to OIM user profile on success of Create User. This update API works perfectly fine when provisioning a user based on Access policy or manually requesting account creation.
However, it can be seen that when an end user request for an Account, after it is approved through SOA approval workflow, the account in target resource gets created for the end user, but the Update User Profile task fails.
