Mediator 11g: Failing to Receive the Response, Exception "Waiting for response has timed out. The conversation id is null"

A Mediator process, which is synchronous, invokes an asynchronous BPEL process. There is a mid-process receive within the BPEL process which issues a callback response to the Mediator process. This should complete the transaction.
Very often, the Mediator process remains in running state until the timeout threshold is reached.
