Web application controls appear grayed out in the ESSO Administrative Console web form wizard (Doc ID 1539710.1)

When trying to create/update a web application template through the web form wizard in the ESSO Administrative Console, all of the fields are grayed out and the username/password fields are not detected or highlighted when selecting the corresponding field name.

The web form wizard is expected to highlight the fields (username/password etc) when selecting the corresponding field names in the wizard.

The issue can be reproduced at will with the following steps:
1. Create a new web application template in the web form wizard of the ESSO Administrative Console.
2. Note that the fields are all grayed out.
3. Click on the fields for the username and password but the fields in the application window do not get highlighted
