FRM-92102 Occurs when Using a SmartCard /CAC (Common Access Card) for Client Certificates

Symptoms
Forms application crashes intermitently with the error:
FRM-92102: A network error or server failure has occurred.
Java console output shows:
Scenario:
- If the access card is in the system when the heartbeat occurs , no error occurs
- If the access card is out of the system when the heartbeat triggered -> the error is generated.
To recover from the error the browser needs to be closed
