Wrong (Old) Host Name Shown in EM for Reports and Forms after Renaming Host

Symptoms
You changed the hostname from Old_Name to New_Name, and attempted to rename Forms and Reports instance according to documentation:

Completed all steps under section:
15.1.2 Changing the Network Configuration of Web Tier Components
Forms and Reports components are not reflecting the new hostname change.
In FMW Control (EM), the following still shows Old_Name as Host:
- Forms
- rep_host_asinst_1
- ReportsTools
- RptSvr_host_asinst_1
- ohs1
Changes
You changed the hostname from Old_Name to New_Name.
