Apache Crashes When Mod_perl Is Enabled On Fusion Middleware 11gR2 (Doc ID 1557612.1)

Enable  mod_perl for Fusion Middleware OHS per:

Oracle® Fusion Middleware Administrator's Guide for Oracle HTTP Server
11g Release 1 (11.1.1)

4 Getting Started with Oracle HTTP Server
4.4.3 Configuring the mod_perl Module

After enabling, the OHS crashes on startup.

Nothing is written to the ORACLE_INSTANCE/diagnostics/logs/OHS/ohs/ohs1.log or  console~OHS~1.log

However, the problem is reported in the Windows Application event_log
