Using Opatch to Apply Patch 16546129 Gets " OPatch failed with error code 73" (Doc ID 1563847.1)

Symptoms

The standalone Opatch is used to apply the upgrade patch, and gets the following error:

Central Inventory: /<HOME_FOLDER>/oraInventory

OPatch version : 11.1.0.0.0
OUI version : 11.1.0.9.0
OUI location : /<MIDDLEWARE_HOME>/oracle_common/oui
Log file location : /<MIDDLEWARE_HOME>/oracle_common/cfgtoollogs/opatch/opatch2013-06-21_14-28-52PM.log

Applying interim patch '16546129' to OH '/<MIDDLEWARE_HOME>/oracle_common'
OPatchSession can NOT load Oracle HOME /<MIDDLEWARE_HOME>/oracle_common Inventory . Possible causes are:
No read or write permission to ORACLE_HOME/.patch_storage
Central Inventory is locked by another OUI instance
No read permission to Central Inventory
The lock file exists in ORACLE_HOME/.patch_storage
The Oracle Home does not exist in Central Inventory

ApplySession failed: ApplySession failed to prepare the system. Unable to create patchObject
System intact, OPatch will not attempt to restore the system

OPatch failed with error code 73
