Forms Migration: Interactive Report Pages with Check Boxes not Functioning (Doc ID 1568784.1)

Symptoms

When a Forms Application migrated to Apex Application, Interactive Report pages with the check boxes are not functioning, and reporting an error. When an Oracle Forms XML file is uploaded to a Forms Migration project, and a user navigates to the Block pages, pg 4400:54 and clicks 'Apply Changes', the same error is reported:
