OAM 11g: System Error Occurs When Credentials Are Submitted in Login Page

Symptoms
Application resources have been protected by Oracle Access Manager (OAM) 11g.
A custom login page has been configured in the OAM authentication scheme that protects the application.
When users access the protected application the custom login page is displayed.
However when the user submits valid credentials the OAM error page is displayed with the OAM System Error message in red.
The OAM server log shows an OAM-02073 error e.g.:
This environment was previously working but various configuration changes have been made recently including configuration of OAM Server for SSL / https access.
Steps to reproduce
1. Access protected application resource.
2. The custom login page is displayed.
3. Submit valid credentials: the OAM Server blue error page is displayed with System Error message in red.
