Exalogic: proxy controller (pc1-vm or pc2-vm) Incident Warning in EMOC -The Asset Can No Longer Be Reached

Information in this document applies to any platform.
The following warning can be seen in EMOC or in EMOC cli incident log file (emoc_cli_incidents) under EC1-vm.
We will also see the following error in the /var/log/messages log for pc1-vm, even though the status of the proxy service is showing as 'online' when checked with "proxyadm status" command.
Additional symptom is that this incident warning will not get cleared even after multiple restarts of the proxy controller or the entire control stack.
