Error "Entry with specified identity was not found" During Extraction Within Forms Recognition (Doc ID 1587791.1)

Symptoms

After importing the vendor pool from an ODBC data source, every batch encounters an error during extraction and ends up in state 250.  The error returned by the RTS instance reads:

Error in classification and extraction step on document 'c:\ofrproject\ap\batches\00000000\22_181_20130829_AD33AF3B4FB34473AF9D12661B65AD14.wdc'. Description: Error extracting class 'Invoices', field 'VendorASSA': Entry with specified identity was not found.
