Created a new forms managed server, WLS_FORMS1, using <Note 989118.1>
Tuning / Load Balancing: How to Manually Create a New WLS Forms Managed Server in 11g (11.1.1.2 and Above) (Doc ID 989118.1)
The new managed server WLS_FORMS1 starts successfully, but forms URL failed with the following error in the browser:
Error 404--Not Found
