Web Tier ( OHS ) 12c Fails to Start - " Invalid Host name : null or empty " / " Invalid instance " (Doc ID 1592480.1)

Symptoms

Oracle Web Tier ( Oracle HTTP Server ) 12c has just been installed and configured as per

Oracle Fusion Middleware Documentation
Installing and Configuring Oracle HTTP Server
Chapter 3 Configuring Oracle HTTP Server in a WebLogic Server Domain 

The Admin Server and Node Manager have been started successfully. And the WebLogic Server Console and Oracle Enterprise Manager Fusion Middleware Control are accessible.

Starting the Web Tier via Fusion Middleware Control with the message  "Operation Failed: Status is SHUTDOWN". Viewing logs reveals the AdminServer is throwing the error "Invalid Host name : null or empty". See screenshots below:
