505 Error When a Servlet Is Called From Server-Side Virtual Include (SSI) In SHTML Page (Doc ID 1593019.1)

Symptoms

After upgrading to WLS 12.1.1, all Server Side Includes (SSI) requests from Apache fail with HTTP error code 505. The wlproxy.log shows the following:
