ADF_FACES-60096 Error While Creating a JVM Controller in EM FMW Control

Using Oracle Forms and ADF runtime with patches, as described in <Note: 1328698.1> .

When attempt to create a Forms JVM Controller in Enterprise Manager, the  following error occurs:

java.lang.RuntimeException: Unable to evaluate #{emas_model_forms_FormsJVMControllerInfo}
  at oracle.sysman.emSDK.model.util.EMObjectUtil.evaluateBareExpr(
  at oracle.sysman.emSDK.model.util.EMObjectUtil.getMBInstancesForCurrentView(
  at oracle.sysman.emSDK.model.util.EMObjectUtil.getMBInstancesForCurrentView(

The issue can be reproduced  with the following steps:

1. Open FMW Control / EM using https://hostname:7001/em
2. Navigate to Farm_Production -> Forms -> forms -> select JVM Controllers
3. Pop-up errors like the following occur:
   Unable to evaluate #{emas_model_forms_FormsJVMControllerInfo}
