ADF_FACES-60096 Error While Creating a JVM Controller in EM FMW Control

Symptoms
Using Oracle Forms 11.1.2.1.0 and ADF runtime 11.1.1.6 with patches 11.1.2.4.0, as described in <Note: 1328698.1> .
When attempt to create a Forms JVM Controller in Enterprise Manager, the following error occurs:

java.lang.RuntimeException: Unable to evaluate #{emas_model_forms_FormsJVMControllerInfo}
at oracle.sysman.emSDK.model.util.EMObjectUtil.evaluateBareExpr(EMObjectUtil.java:401)
at oracle.sysman.emSDK.model.util.EMObjectUtil.getMBInstancesForCurrentView(EMObjectUtil.java:241)
at oracle.sysman.emSDK.model.util.EMObjectUtil.getMBInstancesForCurrentView(EMObjectUtil.java:194)
STEPS

The issue can be reproduced with the following steps:
1. Open FMW Control / EM using https://hostname:port/em
2. Navigate to Farm_Production -> Forms -> forms -> select JVM Controllers
3. Pop-up errors like the following occur:
Unable to evaluate #{emas_model_forms_FormsJVMControllerInfo}
Changes
Installed Forms and Reports 11.1.2.
Installed ADF runtime library by following <Note: 1328698.1>
