OIM11g: Empty Approval Notification Emails or Emails contain "Error 500--Internal Server Error"

Email notifications from tasks in an Oracle Identity Manager (OIM) SOA Approval Composite are empty, or the body of the email can contain:
Where APPROVERX is the recipient of the email notification.
This issue is seen in 18.104.22.168.x or 22.214.171.124.x environments which use an OIMAuthenticator (instead of the out of the box OIMAuthenticationProvider).

One of the steps to implement this configuration is to remove the out of the box OIMAuthenticationProvider and configure an OIMAuthenticator and OIMSignatureAuthenticator.
