Tab on Production Site not Working Properly

Symptoms
HA environment.
A tab is not working properly on a production site, whereas it works as excepted on both the test and development environments.
The tab is displayed on screen, but a new taskflow inside the tab is not being displayed.
On screen the tab item can be seen, but not the region.
The tab access doesn't require different permissions to other tabs.
