ORA-12154-TNS Could not Resolve the Connect identifier Specificed when Connecting from Forms Builder

Information in this document applies to any platform.
Successful access to the Database from sqlplus from an ORACLE_HOME other than the Forms ORACLE_HOME is possible.
Oracle 11g Forms builder cannot connect to the database.
In Windows "Start" --> go to "Oracle Classic Instance - <Instance Name>" ---> "Developer Tools" - Select "Forms Builder"
In Forms builder, select "File" --> "Connect"
In the connect pop-box, enter the same user name, password and database name which was used previously in sqlplus as follows:
User name: xxxx
Note: The same error occurs when attempting to connect to the DB using the sqlplus from the Forms ORACLE_HOME/bin also.
Installed a new database or this is a new installation of Forms and Reports that has not been configured to connect to a remote DB.
