Symptoms

A Coherence*Extend client hangs for a while and then runs into the following exception after a Coherence*Extend proxy failover event:

java.util.ConcurrentModificationException
  at java.util.AbstractList$Itr.checkForComodification(AbstractList.java:372)
  at java.util.AbstractList$Itr.next(AbstractList.java:343)
  at com.tangosol.net.RefreshableAddressProvider.getNextAddress(RefreshableAddressProvider.java:100)
  at com.tangosol.coherence.component.util.daemon.queueProcessor.service.peer.initiator.TcpInitiator.openConnection(TcpInitiator.CDB:28)
  at com.tangosol.coherence.component.util.daemon.queueProcessor.service.peer.Initiator.ensureConnection(Initiator.CDB:11)

The Coherence*Extend client does not recover from this blocked state.
