ESSO-PR Is Failing To Unlock Users Account and Generating Error Message "Unknown Failure" (Doc ID 1609378.1)

Users attempt to use ESSO-PR to unlock account, however cannot do so despite the fact that they have successfully enrolled at an earlier time.  Amongst other messages generated, the message "Unknown Failure" is predominantly seen.
