Oracle Traffic Director - Why Many "health-check reports: all servers in origin server pool origin-server-pool-1 are offline." Errors Along With Many "(Remote I/O error)" in OTD logs ? (Doc ID 1611408.1)

Symptoms

OTD fails with Gateway errors shown in browser.

The OTD server.log file is filled with errors like below:
