Unable to See the Human Task in the Email Body (Doc ID 1615416.1)

Symptoms

You created a human task and task form using 'human task for email'. In notifications tab of human task, you have enabled: make notifications actionable, Send task attachments with email notifications, and make sure "Securing Notifications to Exclude Details" is unchecked.

When you initiate the application, you see the task form in BPM Worklist but it doesn't come in email body. All you see in the email is a line stating: task needs your attention, and approve/reject button. 

Notice the following error in log:
