OAM Form Login Using IIS WebGate Gives 403 Forbidden Error On Submit Of Credentials

OAM WebGate has been installed with IIS 7 and the latest Bundle Patch installed.

An application has been protected with OAM form-based login.

When the credentials are posted from the login form to webgate.dll a 403 Forbidden error occurs.

The server is running IIS7 integrated mode.

When the IIS mode is changed to "classic" the login is working but the application is not able to run.

The WebGate diagnostic page access also generates 403 error: http(s)://hostname.domain:port/access/oblix/apps/webgate/bin/webgate.dll?progid=1

Steps to reproduce
1. Access the protected IIS resource.
2. The OAM login page is displayed.
3. Submit valid credentials, the following error is displayed:
