Portlets can't be Viewed in Preview Mode After Upgrade (Doc ID 162499.1)

Symptoms

Attempting to preview a portlet will raise the error:

404 Not Found, The requested URL
/pls/portal30/portal30.wwpob_page.render_portlet_screen was not found on this server.

Reviewing the error_log will show:

[<TIMESTAMP>] [warn] mod_plsql: Stale Connection due to Oracle error 1036
[<TIMESTAMP>] [error] mod_plsql: /pls/portal30/PORTAL30.wwpob_page.render_portlet_screen ORA-1036 
ORA-01036: illegal variable name/number
