Opening a Word Document from a Download Link Opens Document with Filename of "apex_util.get_blob" When Attachment is Inline (Doc ID 1628760.1)

Symptoms

When using IE9 with Apex through the Apex Listener the application has a document (BLOB) link in a screen.  When clicking on the download link the appropriate options are shown:
Save, Save As and Open.

Save and Save As work as expected. When you open file the file opens with name of apex_util.get_blob and a full name something like the following instead of the actual file name:

http://<servername>:<port>/apex/apex_util.get_blob?s=7563446686644263&a=114&c=2601460389027598234589&p=44&k1=18012&k2=&k=CR

The problem is specific to Internet Explorer 9 and the APEX Listener. When trying to open an inline attachment through APEX, the request should reflect the actual file name.
