When you create a blog post and upload an image there using the "embedded image" function, this image will be stored in inside the Blog folder.
When you now create another blog post, even with another user and upload another, DIFFERENT image with the SAME name, the old image file gets overridden (a new revision is created in UCM). This causes, that new version of the embedded image is shown in all other blog posts.
As this problem occurs with different users, which leads to very bad side-effects, as users might not now that an image with the same name already exists and that those images get replaced.
The same problem also can be reproduced using the Wiki.
