Username field is missing in Create New CSR Case Window (Doc ID 1641576.1)

Symptoms

Unable to create a new CSR case because the username field is missing from the Create New CSR Case window. Redeployment of OAAM Admin does not help.

Changes

 Upgrade to OAAM 11.1.1.5.2 (11g PS1 BP02)
