Errors "TCM-00787: A repository error occurred." and "Unable to create annotation document." Occur when Adding Second Redaction to an Image (Doc ID 1643730.1)

Symptoms

After Upgrading to 11.1.1.8, users are not able to save more the one redaction to a Imaging document.

Users receive the error:

Error Updating Annotation

The Imaging logs show the following error:
